Since the 1920s, the post-apocalyptic world has had widespread access to penicillin, making it an effective treatment for bacterial infections such as the one on Joel's wound.
Ellie is not a licensed medical practitioner, although the transaction was profitable and eventually spared Joel's life. Her medical education is limited to performing only a few sutures here and there, and she doesn't even have Google to consult for guidance when she needs it. She goes as far as waking up Joel hoping he may guide her on where to implant the needle, but her efforts are fruitless. Ellie makes a spur-of-the-moment decision to wing it and administers the penicillin by injecting it directly into the incision.

I'm just thankful that she didn't screw that up. While administering penicillin, intramuscular injections in the buttock or upper thigh should be preferred whenever feasible.
