Episode 6 of The Last of Us Season 1 begins with a flashback to the final moments of the previous episode, when Henry committed himself, before cutting to three months later. This is a tricky method for getting the plot where it needs to be. The arrival of winter weather suggests that Joel and Ellie have made good progress toward their destination of Wyoming in search of Tommy. The camera follows an elderly guy as he goes on a rabbit hunt.
We witness him pull up to his quaint house and open the door to discover Joel pointing a pistol at his distraught wife. Suppressing his pistol, he settles down to listen to what Joel says. Joel asks the man where they are located by pointing to a map and telling him his response had a better match than his wife's. For some reason, Joel puts down his weapon. It's freezing outdoors, so the woman feeds Joel (and Ellie) some soup, which they discuss in a humorous dialogue.

To find his brother, Joel asks the man about the best route to the west. Just in time to hear about the "river of death," as Ellie puts it, Ellie comes racing down the stairs. According to the guy and his wife, Tommy is no longer in the area if he has crossed the river to the west. They frequently come upon corpses and not always sick ones. Joel slows down and puts his hand on his heart as they depart.
Ellie inquires if he is terminal, adding eloquently that if he passes away, she will be doomed. The situation with Joel works out in the end, but we may anticipate future problems. She assures him the only difficult part will be fording the River of Death. They reach a river and Joel declares that it is getting too dark to continue, so they stop to camp for the night. They camp in a cave, where Ellie wonders aloud what Joel plans to do if the fireflies successfully create a cure from her blood. After giving it some thought, he enjoys managing a sheep ranch because the animals are peaceful and obedient.
This works brilliantly as a running gag throughout the program. Ellie goes on to inform Joel that she tried to treat Sam's bite with her blood, but it was unsuccessful. She tells Joel that she and Marlene can develop a cure if they believe they can since Marlene claimed they could. He promises Ellie he'll cover both shifts so she can get some rest, but he promptly passes out. If he does awake, Ellie will be on duty.

 

She says she followed his instructions to the letter and got up early to protect them. After they don't see any infected people emerging from the river after being shot at, Ellie and Joel decide to cross the bridge. Ellie stops and thinks, "Wait, maybe this river is the river of death" as they pass a water mill and dam. Others on horses begin to encircle them almost instantly after that.
They let a dog sniff them to determine whether or not they are contaminated. Joel and Ellie are worried that the dog may feel anything unusual about Ellie, but the dog seems OK with Joel. Not at all. The folks listen as Joel discusses his search for his sibling. When the woman inquires about his identity, he replies, "Joel," and they are sent back to the main camp.
